Pioneer Medical Center is a 25-bed critical access hospital, essentially the county hospital for Sweet Grass County, Montana: Population 3700. We are in a beautiful valley along the Yellowstone River, between three mountain ranges, just north of Yellowstone National Park. We serve our neighbors (ranchers, skilled trades people and active retirees) and the many tourists who come here on their way to Yellowstone and Glacier National Parks, or to fish the Yellowstone, hike, bike, hunt or just get away from overcrowded places. Pioneer Medical Center has an immediate opening for a full-time Outpatient/Clinic/Cardiac Rehab Nurse to join our hospital, emergency & clinics' patient care team.
PURPOSE/DISTINGUISHING CHARACTERISTICS:
Under general supervision, utilizes the nursing process of assessment, planning, implementation and evaluation, and theoretical/clinical skills in the promotion/maintenance of health. Incorporates evidence-based nursing practices consistent with national standards and which support quality outcomes. Assumes responsibility and accountability for the quality of nursing care rendered.

Overview:
We are seeking a highly skilled and compassionate Nurse (RN/LPN) to join our team. This multifaceted role involves providing care in the outpatient clinic, leading the outpatient procedure program, coordinating our cardiac rehabilitation program, and serving as a liaison for Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs. The ideal candidate will have excellent clinical and organizational skills, a passion for patient care, and a commitment to improving health outcomes in our community.
