What are the responsibilities and job description for the Social Services Director position at Pisgah Valley Retirement Community?
We are looking for an experienced Social Worker to join our team for our skilled nursing center.
The candidate is responsible for coordinating and directing Social Services in accordance with federal, state and local regulations.
Qualifications:
- Bachelor's degree in social work, sociology, psychology or human services field
- 1-3 years of experience in Rehab or Long Term Care Setting
- Ability to communicate at all levels
- Problem solving skills
- Excellent organizational skills
- Compassion and desire to work with the elderly
- Positive attitude
- Attention to detail
- Well developed customer service
- Ability to initiate and foster relationships with medical providers, social workers, discharge planners and individuals working in related field
Responsibilities:
- Assist residents and families with social, emotional and family issues
- Coordinate care plan meetings with interdisciplinary team, resident and family
- Coordinate discharge planning
- Provide general information and support to residents, families and general public
- Make referrals for support services when needed
- Resident interviews to assist in the completion of required assessments
Job Type: Full-time
Pay: $48,000.00 - $54,000.00 per year
