General Summary / Responsibilities : The Care Coordinator functions as a member of a multi-disciplinary care team providing services for persons with analcohol and / or other drug Substance Abuse Disorder diagnosis. The primary responsibilities of the care coordinatorinclude : 1) Support adherence to the patient’s treatment and recovery plan; 2) Manage successful care transitions;3) Engagement in care; 4) Services may be delivered by phone, telehealth, face-to-face at the office, in the home, orin the community.
