The Licensed Practical (LPN) is an integral part of the care team that helps support the lives of our patients and residents each day.
Responsibilities Include:
- Provide bedside nursing
- Care planning modifications and implementation
- Managing Patient Care provided by CNAs
- Communicate with IDT members and providers
- Medication administration
- Provide treatments
- Provide information and compassion to residents and family members
- Completing incident reports as needed and participating in investigations
- Documenting accurately in the resident record
- Maintaining organization of the nursing station and medication/treatment carts
- Graduate of approved Licensed Practical Nurse (LPN) Program.
- Previous experience as an LPN in an acute care/long-term care setting is desirable.
- Current licensure to practice as a Licensed Practical Nurse in the State of New York.
