Job Description:
The Patient Care Coordinator manages the nurse/client relationship. The Patient Care Coordinator is responsible for understanding the staffing needs of each client family to understand the client’s staffing preferences. The Patient Care Coordinator is also responsible for managing and developing relationships with our nurses. The Patient Care Coordinator must work closely with our nurses to understand their staffing desires and limitations to best match our nurses with our clients. The Patient Care Coordinator must provide exceptional customer service to both our clients and our nurses.
In addition to managing our nurses and providing excellent customer services to our clients, the Patient Care Coordinator is responsible for posting ads, recruiting, interview, and hiring / reactivating nurses to join our healthcare field staff. The Patient Care Coordinator will work closely with our clinical supervisors to manage our healthcare field staff and ensure all employees are appropriately following company policies & procedures.
PATIENT CARE COORDINATOR RESPONSIBILITIES:
Consistently provides excellent customer service to our patients and their families
Schedules and staffs nurses to patients/families based on patients preferences and the coordinator’s knowledge of our nursing staff
Advises our nurses on how to consistently provide top-tier customer service
Recruits, posts ads, interviews, hires, and rehires nursing staff
Accompany our clinical staff on customer service visits to patient’s homes
Ensures that our staff are utilized as best as possible in meeting our company’s staffing utilization goals
CARE COORDINATOR QUALIFICATIONS:
Strong interpersonal skills with the ability to conduct business and customer services calls and visits to patients/families
Highly effective communication skills in person, on the phone, or via email
Computer literate with the ability to learn a variety of software tools
Proactive attitude that is solutions oriented
Ability to remain calm and perform in a fast-paced work environment
Superior organizational skills with the ability to multi-task
Bachelor's degree or relevant industry experience required
Fluency in English required, Bilingual preferred
PRIME HOME HEALTH Specific Coordinator Tasks:
Actively Posts ads online and on job boards
Recruits and conducts Interviews for new nurses for open positions
Posts nurses’ notes into the Electronic Medical Records System
Calls families to schedule nurses Calls nurses to schedule visits
Conduct patients meet and greets and admissions
Conducts sales calls to grow patient census
Job Type: Full-time
Pay: $20.00 - $23.00 per hour
Expected hours: 40 per week
