Participates in direct patient care and other supportive activities as per patient plan of care under the supervision of an RN. Maintains and applies principles of safety. Collaborates with physician, RN care manager and other ordered disciplines as indicated to maximize patient outcomes. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. ESSENTIAL FUNCTIONS: Provides care as directed on the individualized plan of care to achieve desired patient outcomes for hospice and homecare patients newborn thru geriatric. Performs IV therapy per LPN IV Therapy policy. Observes the patients’ physical, mental, emotional, and spiritual conditions and reports any changes to the RN case manager. Communicates patient assessment and current plan of care with other hospice house staff and volunteers on a routine basis. Collaborates with physician, patient/family and other assigned disciplines regarding care needs and revisions to plan of care. Completes timely and accurate documentation as per agency policy. Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources. Promotes patient’s independence by establishing patient care goals and including the patient and family in discussions regarding medical issues, testing, medications, and self-care skills. Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI). MINIMUM KNOWLEDGE AND SKILLS REQUIRED: Knowledge of MS Office software. Keyboarding skills. Demonstrated ability to communicate effectively. Multitasking and prioritization skills. Ability to apply basic nursing principles. Must be able to lead and/or facilitate process improvement. MINIMUM EDUCATION AND EXPERIENCE REQUIRED: Graduate of an approved LPN program. Successful completion of an Intravenous Fluid Therapy course and passage of the IV Therapy Exam. PREFERRED EDUCATION AND EXPERIENCE: Previous experience in hospice or homecare. REQUIRED LICENSE/REGISTRATION/CERTIFICATION: Current licensure as an LPN in the State of Kansas.
