Promptcare is seeking a highly motivated and experienced Clinical Nurse Liaison to join our healthcare team. The CNL grows core infusion therapy services through referral source/physician relationships, provides customer service to assigned accounts, and provides in-hospital and in-home nursing services to our patients. The ideal must be skilled at helping the referral sources and physicians identify good candidates for home infusion care and must be equally skilled at identifying patients who are not suitable for care in the home. Requirements
- Must be a Registered Nurse in the state inquiring employment and in good standing. If currently only licensed in one state, must be willing to get cross-licensed at the company’s expense
- Must have a valid driver’s license and automobile insurance in good standing
- At least 5 years’ experience as a home infusion nurse, or equivalent experience working in another field-based nursing organization, is required
- Specific training and/or certification as a heart failure nurse is strongly preferred
- Previous experience working with hospital and/or physician office referrals is strongly preferred
- Must have adequate PC skills to use e-mail and update nursing notes and other documentation electronically in a timely manner
- Strong work ethic and desire to serve our referral sources and patients and to grow the business
Job Responsibilities, included but not limited to:
- Evaluates referral source and patient experiences for opportunities to improve customer satisfaction
- The CNL collaborates with referral sources, the patients/family members and PromptCare’s Intake and Nursing teams to facilitate patient discharge and coordination of care
- Teaching the patients/family members about their therapy and supplies and performing the initial hook-up of patients to their IV therapy to facilitate discharge to the home are also critical elements of the job
- Provides the referral sources, physicians, PHI salespeople and patients/family members with timely and accurate information and takes appropriate corrective actions regarding our services to ensure complete satisfaction
- Plans daily events to be as efficient as possible with time management (e.g., minimizes driving time by planning trips ahead of time)
- Ability to properly triage problems in the field in accordance with company standards and other verbal and written training policies
- Follow proper procedures for accurate and timely documentation of referral sources or patient concerns, and most importantly, take appropriate corrective actions
- Maintains strong knowledge of PromptCare’s Standard Operating Procedures (SOPs) and ensures that they are applied consistently
- Maintains compliance with all applicable laws, regulations, and SOPs at all times
- Conducts activities with a mindset of delivering “white glove” service to the referral sources and patients/family members

Physical Demands

This position is active and requires standing, walking, bending, kneeling, stooping, crouching, crawling, twisting, and climbing during the workday. The employee must frequently lift and/lor move items over 35 pounds. Coordinated fine and gross motor movements and hand/eye movement. Maintain balance from any position. Negotiate level surfaces, ramps, and stairs. Function with both hands free for performing psychomotor tasks. Maneuver in small areas. Must maintain cognitive and psychomotor tasks for up to 7-12 hours. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust. This position may require driving up to eight hours consecutively.
