Rainbow Community Care is currently accepting applications for a dedicated, compassionate Admissions nurse. The position could be 32-40 hours a week depending on the candidate.
The Admissions Registered Nurse is responsible for evaluating patients for admission to hospice services, completing the initial assessment, and providing information to patients and families regarding hospice services. The Admissions Nurse gives skilled nursing care to patients in their homes, administers medications and treatments, addresses the patients’ most immediate care needs, and prepares the patient to be followed by their primary hospice team.
Essential Duties:

Conduct an initial assessment of the patient and their family. Completing all other documentation that is necessary in the patient plan of care.

Provides interventions for immediate care needs and refers to other team disciplines as necessary to meet immediate care needs.

Provides guidance to referral sources and patients and families about the benefits of hospice services.

Ensure that the nursing needs of the patient are met as identified in the patient’s initial assessment, comprehensive assessment, and updated assessments.

Able to document concisely data that supports eligibility in the patient electronic record.

Knowledgeable about the 4 levels of hospice care, Rainbow processes, eligibility and documentation requirements.

Supports nursing department for visit needs and staffing as requested by supervisor.

Implements physician orders and maintains communication with primary care physician.

Teaches family members’ patient care skills, meds, treatments, use of equipment and informs them of signs and symptoms of dying and grieving process.

Obtains verbal certification of terminal illness from attending physician and hospice physician.

Develops nursing diagnosis and care plans individualized for each specific patient and modifies care plan based on ongoing assessment of patient condition.

Ability to drive to and from patients’ homes and to meetings at other locations as necessary.

Professional Qualifications

Active WI RN Licensure must be currently maintained.

Valid Wisconsin driver’s license required upon hire and must be maintained.

Valid auto insurance required upon hire and must be maintained.
-
Hospice, palliative care, home health experience preferred.
