COMMUNITY LIVING SUPPORT SPECIALIST (NON-QMHP)

GENERAL DEFINITION OF WORK: FLSA Status: Non-Exempt

Provides general support/monitoring activities while working with clients to enhance living skills. Provides housekeeping, personal assistance and transportation services to clients; does related work as required. Work is performed under regular supervision.

ESSENTIAL FUNCTIONS/TYPICAL TASKS:

Supporting clients with living skills; providing personal care to clients; performing light housekeeping; providing transportation; Maintaining all necessary documentation

· Supports clients with daily living skills and provides assistance and supervision to clients; provides crisis intervention services.

· Provides personal and environmental care tasks such as housekeeping, health care, transportation, home management, nutrition and shopping to clients; maintains a clean, comfortable and safe environment.

· Performs environmental and home management tasks including vacuuming, dusting, mopping, sweeping, cleaning bathroom and kitchen, changing linens, laundry, meal preparation, transportation and shopping.

· Assists with budgeting; maintains doctor’s appointment schedule and prescription refills; dispenses medication as directed by physician

· Monitors and supports tasks from client-specific service plan.

· Serves as liaison between roommates, emergency workers, and other community resources; confers with medical and mental health providers to ensure client receives appropriate care.

· Motivates client toward self-sufficiency and esteem while respecting privacy and dignity of client and exercising considerable tact and independent judgment in dealing with client and client’s family.

· Supports clients in demonstrating independence, self advocacy and coping skills.

· Attends training sessions and meetings; provides updates to QMHP or Community Living Support Supervisor.

· Performs related tasks as required.

KNOWLEDGE, SKILLS AND ABILITIES:

Thorough knowledge of basic personal health care and home management methods; thorough knowledge of behavior management and crisis intervention; Supports clients with Mental Illness, Intellectual disabilities or a dual diagnosis of both; ability to support clients with basic healthcare and home management methods; ability to communicate ideas effectively both orally and in writing; ability to establish and maintain effective working relationships with clients, landlords, community resource providers and associates; knowledge of MS office to include word and Excel.

Must be a team player, dependable, have the ability to adapt in stressful situations; must be flexible, pay attention to detail, be computer efficient, have creative and innovative thinking; must be diverse and client focused, have excellent communication skills both oral and in writing, must have excellent multi- tasking skills, organizational skills and meet deadlines.

EDUCATION AND EXPERIENCE: High School Diploma and at least providing direct behavioral health services to adults with mental illness, intellectual disability or dual diagnosis.

PHYSICAL REQUIREMENTS:

This work requires light physical effort working almost exclusively with lightweight materials (up to 25 lbs) or short periods in difficult work positions; working conditions are good; may involve occasional exposure to some disagreeable elements and accidents are improbable; health hazards negligible; some mental effort and stress may be involved resulting in inconvenience and frustration; the worker is subject to inside and outside environmental conditions. The worker may be exposed bloodborne pathogens and may be required to wear specialized personal protective equipment.

SPECIAL REQUIREMENTS:

Possession of an appropriate driver’s license valid in the Commonwealth of Virginia. Attends training and maintains certifications as required by the Agency and regulatory guidelines. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential tasks.
