Reasons Eating Disorder Center (Reasons) is seeking candidate professionals, preferably with previous experience working with eating disorders, trauma, substance use and LGBTQIA communities, to provide compassionate care to our diverse patients for our residential treatment facilities located in beautiful, intimate six-bed houses in the Pasadena, San Marino, and the Arcadia neighborhoods of the Los Angeles area.
The Case Manager for Reasons Residentials is a key member of the Residential leadership team, responsible for managing the discharge planning coordination offered to the Reasons patients, which may include assessment of new admissions, clinical input into the establishment of treatment plans, interfacing with the treatment team/external case managers, authorizations, and utilization review with managed care organizations. This position provides oversight and/or implements and accomplishes all aspects of the discharge planning process. This position will provide therapy groups to include aftercare planning, relapse prevention, and life skills. This position will attend treatment teams, document in and audit medical records, meet with patients weekly to coordinate discharge planning and social work needs.
Requirements:
Education: Masters, Doctorate Degree or RN from an accredited college or university in social work, mental health, psychology or a related field required.
Experience: A minimum of four (4) years direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, family motivation, treatment planning and communication with external review organizations or comparable license. One (1) year of experience working in an eating disorders treatment setting preferred.
