What are the responsibilities and job description for the RN Hospital Case Manager position at Redington-Fairview General Hospital?
Under the general direction of the Hospital Case Management Director, the RN Case Manager will coordinate the care and services of an assigned patient population, promote effective utilization, monitor health care resources and assume a leadership role with the interdisciplinary team to achieve optimal outcomes with the use of case management tools such as clinical guidelines. The Case Manager will perform a wide variety of patient related activities to include admission review for medical necessity and appropriate level of care, application of clinical guidelines, interaction with physicians, staff, patient/family and outside agencies. The Case Manager is also responsible for daily Utilization Review activities including identification of quality care issues, infection control surveillance, compliance with UR specific regulations and insurance reviews.
Status: Full time, 32 hour
Qualities: Must work well independently with some supervision. Must be professional and personable with a wide range of individuals. Must have great attention to detail.
Requirements: State of Maine RN license with at least 3-5 years of acute hospital experience and preferably one of those years in infection control, utilization review position and/or case management.
