Regal Heights Rehab & Health Care Center is Hiring a Social Worker Near Queens, NY
Our Nursing Home located in Queens NY is seeking a Social Worker to join our Social Services team.
Social Worker
Duties and Responsibilities:
- Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
- Initiating appropriate interventions for residents experiencing mental and behavioral health issues
- Identifying individual social and emotional needs
- Develop and maintains individualized care plans
- Participate in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Qualifications/Requirements:
- BSW or MSW preferred, or Bachelors degree in related field
- At least one year experience in Social Work field required
- Experience in long term care and/or sub-acute setting preferred
- Must have experience in Minimum Dates set 3.0(MDS) Assessments and use of EMR for charting
