Responsibilities include : completing designated intake screenings and assessments for services. Provide supervision to two designated clinicians. Makes appropriate referrals internally and for other community services when needed. Acts as clinical liaison to assure effective coordination of clinical services between Reliance Health programs and area clinical providers, emergency departments, and inpatient providers regarding treatment, advocacy, and admission / discharge planning. Reviews designated clinician’s documentation including notes requiring signature.
