Introduction:
We are seeking a dedicated and compassionate Chronic Care Management Medical Assistant (MA) to join our healthcare team. The ideal candidate will assist in providing comprehensive care for patients with chronic conditions, working closely with the healthcare team to manage and improve patient outcomes. This role will involve both administrative and clinical duties to support chronic care management programs.

Key Responsibilities:

  • Patient Support: Assist in coordinating care for patients with chronic conditions (such as diabetes, hypertension, COPD, heart disease) by performing routine checks, health assessments, and providing education on disease management.
  • Care Coordination: Schedule and follow up on patient appointments, laboratory tests, and other diagnostic procedures. Ensure patients adhere to treatment plans and help them manage their conditions through regular check-ins and follow-ups.
  • Health Monitoring: Record and monitor patient vital signs, symptoms, and progress. Track health data and assist providers with the management of care plans.
  • Documentation: Maintain accurate and up-to-date patient records, documenting all interactions and care plans in the electronic health record (EHR) system. Ensure compliance with healthcare regulations.
  • Patient Education: Provide education to patients and their families on managing chronic conditions, medication adherence, and healthy lifestyle choices. Answer patient questions and provide resources as needed.
  • Collaboration: Work closely with physicians, nurses, case managers, and other healthcare professionals to ensure comprehensive care for patients. Communicate effectively within the healthcare team to promote patient outcomes.
  • Administrative Tasks: Perform office-based administrative duties such as handling phone calls, confirming appointments, insurance verification, and patient intake.

  • Chronic Disease Management: Specialized in managing patients with long-term chronic conditions such as diabetes, hypertension, asthma, and cardiovascular diseases, ensuring comprehensive care plans tailored to individual needs.
  • Care Coordination and Follow-up: Expertise in monitoring patients between visits, ensuring that patients receive necessary treatments, tests, and follow-ups as part of their care plans.
  • Patient Education and Empowerment: Skilled in providing patients with the knowledge and tools needed to effectively manage their conditions and make informed decisions about their healthcare.

Qualifications:

  • Education: High school diploma or equivalent; completion of a Medical Assistant program (certification preferred).
  • Experience: Prior experience in chronic care management, medical assisting, or a healthcare setting is preferred.
  • Certifications: Medical Assistant Certification (CMA, RMA, or similar) preferred.
  • Skills:
  • Strong communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Knowledge of chronic conditions and care management techniques.
  • Proficient with Electronic Health Records (EHR) and basic computer software.
  • Ability to work both independently and as part of a team.

Physical Requirements:

  • Ability to stand, sit, and walk for extended periods.

Job Type: Full-time

Pay: $14.00 - $18.00 per hour
