We are hiring a Licensed Practical Nurse (LPN) for Hospice in the Medford and Grants Pass area.

Position: Hospice Licensed Practical Nurse (LPN)
Type: Full-Time

Requirements:

• Current Oregon Licensed Practical Nurse license
• BLS Certification
• Licensed driver with an insured, reliable vehicle

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse
• Administer medications and treatments as prescribed
• Monitor patient health and report changes to the RN or physician
• Assist with patient assessments and care planning
• Educate patients and families on health care needs and treatments
• Maintain accurate patient records and documentation
• Ensure compliance with all health regulations and standards
• Maintain a clean and orderly environment for patients
• Other duties as assigned
