Job Summary
We are seeking a compassionate and dedicated Registered Nurse to join our healthcare team. This role involves collaborating with multidisciplinary teams to develop and implement comprehensive care plans, focusing on discharge planning, health coaching, and patient education. The Registered Nurse will also have the opportunity to engage in specialized areas such as esthetic laser treatment and family planning.
Duties
- Assess and monitor patient health status, including vital signs and medical history.
- Develop individualized care plans based on patient needs and preferences.
- Provide direct patient care in various settings, including acute care and senior care.
- Educate patients and families on health management, medications, and treatment options.
- Collaborate with physicians and other healthcare professionals to ensure coordinated care.
- Participate in discharge planning to facilitate smooth transitions from hospital to home or other facilities.
- Document all patient interactions accurately in accordance with ICD-10 coding standards.
- Engage in health coaching to empower patients towards better health outcomes.
- Stay current with best practices in nursing and participate in ongoing professional development.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in internal medicine, acute care, or related fields preferred.
- Strong communication skills with the ability to educate patients effectively.
- Knowledge of discharge planning processes and health coaching strategies.
- Familiarity with esthetic laser treatment is a plus but not required.
- Ability to work collaboratively within a multidisciplinary team environment.
- Compassionate demeanor with a commitment to providing excellent patient care.
- Previous experience in a teaching hospital setting is advantageous.
