Job Description: Federal Case Manager

Position Summary
The Federal Case Manager reports to the Federal Program Director and is responsible for developing and coordinating re-entry programming that ensures continuity of care for residents. The Federal Case Manager will meet with residents, maintain collateral contacts with contractors and community resources, provide documentation of resident activities, attend required meetings/trainings, and reporting. The Federal Case Manager will ensure the requirements of the Federal Bureau of Prisons Statement of Work (SOW), and the Residential Services Department Policies and Procedures are met.
Essential Functions
- Create an Individualized Program Plan with each resident within 10 calendar days of entry, in consultation with the Program Coordinator and Employment Placement Specialist, to address the resident's needs and risks concentrating on, but not limited, to the following areas: housing, employment, financial management, physical health, and education.
- Monitor residents’ behavior directly and through other staff to ensure compliance with all aspects of RVCP and FBOP programming.
- Meet with residents for individual sessions bi-weekly, at a minimum for 1 hour, following the completion of the Individualized Program Plan.
- Meet regularly with other professionals inside and outside the agency to ensure provision of quality care to all residents.
- Initiate and maintain documentation including Intake paperwork, Individualized Program Plans, Progress Reports, Case Notes, Home confinement paperwork, Exit Plans, Incident Reports, and Discharge Letters.
- Audit a resident’s file at the time of the initial individual session and at the time of the final individual session.
- Attend required meetings/training and ensure that Individualized Program Plans, Progress Reports, Termination Reports, Incident Reports, and Discharge Letters are completed within specified timelines and are sent to the appropriate parties for reporting and billing.
- Participate in facility searches when asked to assist by PSS Staff.
- Have regular attendance on the job to complete essential job functions.
- Respond promptly to all calls and messages from RVCP supervisors.
Requirements
- Degree and/or equivalent experience in Human Services, Criminal Justice, or Counseling.
- Be knowledgeable and in daily compliance with the Federal Statement of Work (SOW) and the Community Based Residential Facility (CBRF) licensing requirements.
- Ability to acquire and apply a working knowledge of community support and health resources and how these resources may be used by persons in the Criminal Justice System.
- Ability to relate to people in an unprejudiced and understanding manner with concern for their circumstances and feelings.
- Successfully complete pre-employment state and federal background checks, a pre-employment physical, urine drug test, and TB test.
- Successfully complete required CBRF training.
Physical Demands
The employee is regularly required to talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must occasionally lift and/or move up to 25 pounds.

It is an expectation that Counselors and Case Managers work together professionally to ensure the highest quality of care is provided to RVCP’s clientele. Examples of collaboration include regular discussion regarding shared clients' progress, barriers, or needs; Problem-solving situations/issues; supporting each other and presenting as a united treatment team to all clients. It is very essential that the treatment team supports each other and have honest and open communication that ensures the highest level of quality care for our clients.
