The Community Health Worker (CHW) plays a vital role in bridging the gap between healthcare providers and the community. Responsibilities
- Serve as a liaison between healthcare providers and community members to facilitate access to services.
- Educate individuals and families on health-related topics, including disease prevention, nutrition, and available resources.
- Assist clients in understanding Medicare and other public health programs.
- Provide support for individuals dealing with addiction through counseling and resource navigation.
- Conduct outreach activities to engage underserved populations and promote health initiatives.
- Utilize motivational interviewing techniques to encourage behavior change among clients.
- Maintain accurate records of client interactions and progress in accordance with organizational policies.
- Collaborate with social workers, nurses, and other healthcare professionals to coordinate care for clients with complex needs.
- Participate in community events and public speaking engagements to raise awareness about health issues.
Skills
- Strong background in social work or public health is preferred.
- Knowledge of medical terminology to effectively communicate health information.
- Experience in addiction counseling or working with individuals with disabilities is a plus.
- Excellent public speaking skills for presentations and community outreach efforts.
- Ability to educate diverse populations about healthcare resources and services.

The Community Health Worker is a grant based position and will work directly with Rural Health’s Rural Communities Opioid Response Program (RCORP) Impact grant, Medication Assisted Treatment (MAT) Program and the Social Service Department.
