The Hospice and Palliative care Licensed Clinical Social Worker is a qualified professional, who delivers varied social work services to Hospice, Pre-Hospice and Palliative patients and their families. The LCSW is an integral part of the Hospice IDG who provides initial and on-going emotional, psychosocial, grief and bereavement assessment and support, funeral planning, community education, outreach and referral support and service.
This is a field position visiting patient homes and facilities in the Southern and Central towns of Fairfield County, CT.
Responsibilities : Hospice :
- Perform Initial Hospice Social Work Comprehensive Assessment
- Conducts ongoing reassessments of patient / family needs and counseling as required.
- Provides short term crisis intervention and individual or family counseling when indicated.
- Provides services, under the direction of a physician. Referral to Community resources as appropriate.
- Observes and assesses psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
- Assumes the active role of advocate for the client / family and caregiving unit.
- Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Quality documentation for all client visits, contacts, referral actions, and assessments, including comprehensive psychosocial, emotional and spiritual assessment.
- Provides ongoing counseling related to issues of death and dying to the client, family and caregivers as needed.
- Attends staff meetings, IDG, case conferences, educational programs, and peer consultation meetings as mandated by the State of Connecticut.

Compensation :
Our target pay range for this position is $78,000 to $83,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
