Department: Case Management Works under the supervision of the Director Case Management and/or Clinical Social Work Manager. Provides professional social work services to patients and their families presenting difficult and complex medical and related psychosocial problems; or, in addition to providing direct patient services. Has responsibility for the social work activities of the department. Performs the duties of the Clinical Social Work Manager in their absence. 1. Interviews and evaluates patient and family members to obtain psycho-social history; develops social/emotional assessments and formulates diagnosis and treatment plans based on evaluation of the patient’s past, present, and future medical and social-economic functioning. 2. Develops and implements psychosocial therapy programs for patients utilizing a variety of therapeutic methods and techniques. 3. Provides consultation services to multi-disciplinary team members by evaluating the environmental and psychosocial implications of the patient’s illness when the team is assessing and planning medical treatment. 4. Develops and utilizes resources of public and private agencies and community organizations to meet the needs of the patient. Confers with representatives of other departments and private agencies to resolve problems and to develop and maintain public understanding and sound working relationships. Participates as a member of administrative educational, professional, and community committees for program planning per the Director Case Management and/or Clinical Social Worker Manager’s request. 5. Directs the social work services of the units assigned to and supportive staff. Participates in interviews of Social Worker candidates. Prepares performance evaluations, if necessary, makes case assignments, and reviews work performance during Manager’s absence. Provides technical instructions through regular individual staff conferences as needed. 6. Provides orientation to multi-disciplinary health staff to further their understanding of the Social Worker’s role in the treatment process. 7. May conduct research studies and special projects to develop more effective methods of identifying and evaluating the needs of the patient. 8. Writes case summaries, reports, maintains statistical records, and may submit recommendations on programs, procedures, budget requests, and staffing needs. 9. May develop and conduct in-service training and staff development programs for social work and hospital staff. Participates in social work student affiliate programs for training professional social work students through case assignments and technical instructions. 10. Attends in-service training programs, continuing professional education, seminars, and workshops to keep abreast of new trends and techniques used in social work practice. 11. Should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. 12. Performs other duties as assigned. Education: Master Degree in Social Work (MSW) required. Licensure: Licensed Clinical Social Worker (LCSW) by the California State Board of Behavioral Science Examiners required. Experience: Five (5) years of social work experience in a recognized social service agency, clinic, hospital, or public health agency, involving issues of medical, behavioral health, social services or public health, child or adult protection services; one (1) year of, which must have been in a medical, public health, social services or behavioral health setting. Pay Range: The hourly rate for this position is $49.14 - $65.00.
