Description
Summary
To provide direct support within the day program and to assist direct care providers with increasing behavior skills and behavior plan implementation. To support direct care providers in managing the expectations of the direct care position in alignment with agency values and expectations while also supporting a collaborative environment for progress and growth.
Duties and responsibilities
- Provide direct training to direct care providers through shadowing, behavior plan review, and specific client information and expectation review
- Provide support to direct care providers to support each other on a consistent basis to cope with challenging situations that may arise
- Complete client related paperwork and quality assurance tasks, incident reports, ISSP updates, and other applicable assessments as assigned by Program Manager
- Provide direct supervision and support of clients as related to program and client-specific protocols and needs
- Support on-site transitions and scheduling needs
- Follow agency policies, procedures and protocols to ensure safety and quality care
- Implement positive behavior supports and client-specific behavior plans
- Continually support clients to demonstrate self-determination and independence
- Provide detailed, daily documentation of all time spent on identified activities
- Regular communication with Program Manager to relay critical client-related information
- Provide ongoing progress reports to teams/Sample Supports as requested by Program Manager
- Any staff member must be willing and able to work with any client in any program in any region
- Staff is responsible for maintaining identified levels of client supervision as identified in client service plans
- Staff is responsible for effectively implementing emergency control procedures and safety control procedures as necessary
- Assist in the field as needed
- Be available to assist team members in the event support is needed for client behavior escalations, medical emergencies, vehicle emergencies, or any other unexpected event
- Reliable transportation is required to get to work/drive clients
- Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at- risk individual. (M/A/N/E) Each position is considered a mandatory reporter
- Any other tasks as assigned

Qualifications
The ideal candidate possesses the following characteristics/skills:
- Ability to effectively work independently with minimal supervision
- Must be at least 21 years of age
- Must meet agency requirements to transport clients
- Bachelor’s Degree strongly preferred
- Completion and ongoing competency of all required trainings offered or organized by Sample Supports
- Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained
- History of timely tracking and admin responsibilities
- Able to perform the physical demands of the position
Physical Demands:
Due to the flexible nature of positions and expectations in a team environment, all employees are expected to fulfill the physical demands of each position. Employees are expected to be available to one another, cover shifts, and work together to get the job done. Lifting:
- Employees are expected to lift 75 pounds, depending on specific job site expectations.
Motion:
- Employees are expected to utilize a full range of motion and be able to stand and walk for the duration of their shift (8 hours).
- Employees are expected to physically intervene with clients that pose a safety threat to themselves or others utilizing NVCI-approved techniques.
- Employees may be expected to run/jog for 20-30 minutes if necessary in the event of a client elopement.
- Must be able to lean, squat, and bend in order to appropriately administer appropriate care to client, including administering first aid, and/or CPR for up to 45 minutes.
- Must be able to assist clients with gait issues, offering physical assistance and support in all weather conditions.
- Must be able to lift and/or assist clients in and out of vehicles.
- Ability to be able to work in front of a screen (computer, phone, etc.) for 6-8 hours a day.
- Employees are expected to physically intervene with clients that pose a safety threat to themselves or others utilizing NVCI-approved techniques.
Working Conditions
- Sample Supports' business hours are 24 hours a day, 7 days a week so employees should expect to work some evening and weekend shifts
- Ability to work independently, meet deadlines with minimal supervision, schedule time efficiently
- Must be comfortable working with and around individuals who may exhibit unpredictable and/or volatile behaviors
$23.75 / hourly
Salary : $24
