A Fantastic SNF/Rehabilitation facility is seeking a Social Services Director to join our long-term care team. In this role, you will ensure that the physical, mental, and emotional needs of our residents are met. In order to do this, you will complete initial assessments, implement care plans, and document resident progress.
You will also coordinate family meetings and provide information on resources that are available in the local community. Applicants must have a bachelor's or master's degree in social work or human sciences, as well as state licensure or certification. You should have some experience working directly with people in need of social services, preferably at a skilled nursing facility (SNF).
Social Service Director Duties and Responsibilities
- Ensure the needs of our long-term care residents are met
- Maintain a community resource list
- Complete initial assessments
- Implement care plans
- Assist with discharge planning
- Document patient progress
- Coordinate family meetings
- Ensure regulatory compliance
Social Service Director Requirements and Qualifications
- A bachelor's degree in social work or a closely related field
- State licensure preferred (BSW or MSW)
- Experience with social work, preferably working directly with patients and their family members at a skilled nursing facility
- Interpersonal, critical thinking, and organizational skills
- Focus on Team work and Customer/Resident Service

Pay: $22.00 - $38.00 per hour
