Fee for Service /Per Diem Community-Based Therapist

CFTSS (Children and Family Treatment and Support Services) offers opportunities to better meet the behavioral health needs at earlier junctures in a child/youth’s life to prevent the onset or progression of behavioral health condition. CFTSS are a set of services designed to help youth and families to gain the necessary tools to be successful in their homes and communities through goal directed supports and solution-based interventions and offer a variety of clinical and rehabilitative services provided in the home or other community setting. They are intended to identify the behavioral health needs early and intervene to keep youth on their developmental trajectory to avoid the need for higher levels of care.

Other Licensed Practitioners are Licensed Practitioners of the Healing Arts which includes LMSW, LCSW, LMHC, LMFT, LCAT clinicians. The Other Licensed Practitioners (OLP) operate under the CFTSS umbrella of services to provide clinical and rehabilitative services in the home or other community-based setting in accordance with a treatment plan based on the youth and family’s specific needs. The OLP offers greater flexibility to the youth and family which is expected to engage more effectively those who may have difficulty engaging in a traditional clinic-based setting. The OLP will deliver rehabilitative interventions using a variety of counseling modalities and approaches to promote functional improvement in daily living, interpersonal relationships and community integration, and avoid and manage crises. OLP services are provided using trauma informed, culturally and linguistically competent manner.

The OLP Duties include:

· Licensed Evaluation/Assessment: Identify the youth and family strengths and challenges, identify service needs through a comprehensive assessment and observation of the youth. Diagnoses may be established where necessary under the scope of practice and assessments will be conducted using the NYSCRI tool and be responsible for determining if the youth meets Medical Necessity based on program criteria.

· Treatment Planning: OLP’s will develop the initial treatment plan for the youth that specify the amount, duration and scope of services developed in collaboration with the youth and family. The treatment plan will also document the expected outcome using person centered principles and include a safety plan developed with the youth and family. The treatment plan will be updated every six months or as necessary before the six month expiration point to reflect the updated needs of the youth and family.

· Counseling: Provide individual, family and group psychotherapy as prescribed per the treatment plan. Counseling will be provided as a way of alleviating symptoms of the behavioral health diagnosis and work towards adjusting maladaptive behaviors and encouraging personal growth and development.

· Crisis Intervention: Provide crisis intervention services that are immediate and designed to diffuse or resolve the precipitating stress --in-person or by telephone and with follow-up. Promote crisis avoidance and management counseling to prevent or reduce an active crisis and promote stability after the episode.

· Availability/ Documentation: Complete required documentation for Medicaid billing within prescribed timeframes. Have availability to provide services during after-school/evenings and/or at least one weekend day and maintain a consistent schedule in agreement with the family.

Qualifications:

  • Must have active LMSW, LCSW, LMHC, LMFT or LCAT and be knowledgeable regarding trauma informed care.
  • At least one year of applicable experience in children’s mental health or related area working with youth and families.
  • Follow COVID safety protocols and safety precautions needed to protect youth population
  • Must be willing to work in client’s homes and communities.
  • Must have access to a computer and printer and be able to utilize the computer for documentation into an electronic health record, the printing and obtaining of consent signatures and timely email correspondence with the program.
  • Undergo background checks, fingerprints and driving clearances as required by SCO Family of Services.
  • Evidenced-based practices (EBPs) require approval, designations and fidelity reviews on an ongoing basis as determined necessary by New York State.
  • Must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.
  • Must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
  • Valid Driver's License a bonus
  • Some telehealth permitted at the request of the client/family.
Working Conditions:
 
  • Must commit to 8 billable sessions per week
  • Ability to work at least 2 week nights and Saturday shift
  • Flexible schedule
