Job description
Job Title: Case Aide
Function: The Case Aide works closely with the Care Coordinators with their case management activities.
Reports To: Care Coordination Unit Supervisor, Associate Director, Executive Director
Major Duties and Responsibilities:
· Assist in providing information to SSA participants via telephone regarding care needs and resources.
· Aid the Care Coordinators to compile information pertaining to social, personal, financial, or medical history.
· Assist Care Coordinators with preparation of forms for assessments and implementation of services.
· Maintain files/record for Care Coordinators regarding visits of SSA participants. This includes, but not limited to: typing, copying, scanning, using fax machine, filing, making telephone calls, and completing forms.
· Assist Care Coordinators with reviewing paperwork to ensure all areas of record is completed, billing, and routing of paperwork to providers and follow up of those activities.
· Assist Care Coordinators with entering notes into ECCPIS and Critical Event Reporting
· Assist Care Coordinators with 6-month follow up- precheck list
· Assist applicants with completing required Medicaid/SNAP application on ABE website
· Documents in AgingIS, STARS, completing Daily Service Logs (DSLs), as well as other required documentation.
· Other duties as assigned deemed necessary by supervisor to support the Care Coordinators daily activities.
Key Skills Required:
· Bachelor’s Degree required
· Must be organized, mature and professional
· Will represent the agency in the community by following Senior Services Mission Statement and FISH philosophy.
· Excellent oral and written communications skills, customer service skills
· Able to handle multiple issues simultaneously
· Detail oriented – maintain thorough, accurate records
· Good judgment and decision-making, must be able to prioritize important calls, and tasks.
· Level-headed, able to act/interact appropriately in emotional situations.
· Able to deliver uncomfortable information in such a way as to minimize adverse reactions.
· Must be able to maintain confidences and absolute privacy of SSA participant information.
· Must have computer skills
Work Location:
· May be split between two locations
Schedule:
- Monday to Friday 8:00 am – 4:00 pm; 7.50-hour workdays; half hour unpaid lunch
- Full-time position only; Non-exempt
- Volunteering for agency events is desired
