The Meals-on-Wheels Services Associate is responsible for conducting in-home assessments and reassessments of participants to determine eligibility for the Meals-on-Wheels program, preparing and maintaining required documentation, and addressing participant needs by collaborating with relevant team members. This role requires strong organizational skills, effective communication, and proficiency in documentation, as well as the ability to work with diverse populations, particularly the elderly. The Associate also supports program operations by assisting with intern training, food stocking, and administrative tasks, ensuring a smooth delivery of services to our community’s seniors.
Starting Rate: $35,267 Annually (Non-exempt, salaried)
POSITION SUMMARY
Conducts in-home assessments of potential Meals-on-Wheels participants and reassessments of
current participants in the nutrition program, prepares required documentation, communicates
participant issues and concerns to the appropriate staff and/or agency.
QUALIFICATIONS
Education, Experience, Training:
Four year degree in social or health sciences. Experience working with people of diverse
backgrounds and the elderly, a plus.
Knowledge, Skills, Ability:
- Excellent organizational and multi-tasking skills
- Effective communication and documenting skills
- Computer skills with Microsoft Office
- Ability to collaborate with others
- Good driving skills
Physical Requirements:
Sedentary work with some driving and lifting. Repetitive motion, close visual acuity, talking,
listening, and writing. May be exposed to adverse environmental conditions during participant
home visits.
ESSENTIAL FUNCTIONS
75% Conducts in-home assessments and/or reassessments and completes paperwork to
determine participant eligibility for program services; gathers information to complete required
program documents; reports additional participant issues or makes referrals as indicated; obtains
participant signatures as required and completes all documentation; updates participant
information in SERVTracker as necessary; may provide driving directions for new participants and
assist with weekly paperwork.
25% Collaborates with other MOW Services staff members and performs additional participant
services as needed; assists with the training of interns; assists with picking up and stocking food
and other items in support of program operations/activities; other duties as assigned.
SUPERVISORY RESPONSIBILITIES
Responsible for own work only.
AUTHORITY/ACCOUNTABILITY
Accountable for timely and accurate completion of assessments and reassessments, and
appropriate communication within the department. Responsible for compliance with HIPAA.
PROBLEM SOLVING
Solves problems relating to assessment, eligibility guidelines, scheduling, and NCDAAS
compliance.
