Job Overview
We are seeking a dedicated and compassionate Intake Registered Nurse (RN) for our Home Health department. Responsibilities
- Conduct comprehensive patient assessments, including medical history and physical examinations.
- Develop and implement individualized care plans based on patient needs and preferences.
- Provide direct patient care, including sterile processing, catheterization, and wound care.
- Monitor patients' health status and respond to changes in condition with appropriate interventions.
- Document all patient interactions and medical information accurately in medical records.
- Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes.
- Educate patients and families on health management, medication administration, and lifestyle modifications.
- Perform diagnostic evaluations as needed to assess patient progress.
- Support patients with behavioral health needs through compassionate care.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in acute care or critical care settings preferred.
- Strong skills in medical documentation and record keeping.
- Experience in primary care or home health nursing is a plus.
- Ability to perform catheterization and other specialized nursing procedures.
- Excellent communication skills, both verbal and written.
