Job Summary
The Case Manager plays a pivotal role in coordinating patient care and ensuring that individuals receive the necessary medical services throughout their treatment journey. This position requires a compassionate professional who can effectively communicate with patients, families, and healthcare providers. The Case Manager will assess patient needs, develop care plans, and facilitate access to resources, all while maintaining a focus on quality care and patient satisfaction.

Responsibilities

  • Conduct comprehensive assessments of patients to determine their medical, psychological, and social needs.
  • Develop individualized care plans that address the unique requirements of each patient.
  • Collaborate with healthcare teams to ensure seamless communication and coordination of services.
  • Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
  • Educate patients and families about treatment options, resources, and discharge planning.
  • Maintain accurate medical records and documentation in compliance with regulatory standards.
  • Advocate for patients' rights and preferences within the healthcare system.
  • Utilize knowledge of medical terminology and physiology to inform decision-making processes.
  • Engage in utilization management to optimize resource allocation and improve patient care efficiency.

Skills

  • Proficient understanding of medical terminology and its application in clinical settings.
  • Strong knowledge of physiology to comprehend patient conditions effectively.
  • Experience in patient care practices within various healthcare environments.
  • Familiarity with managing medical records and ensuring confidentiality.
  • Expertise in discharge planning processes to facilitate smooth transitions from hospital to home or other care settings.
  • Experience working in a Level I Trauma Center or critical care environment is highly desirable.
  • Ability to conduct utilization management assessments to enhance service delivery.
  • Competence in monitoring vital signs and understanding their implications for patient health.
  • Knowledge of primary care principles to support holistic patient management.

This role is essential for enhancing the quality of healthcare delivery while ensuring that patients receive comprehensive support throughout their treatment process.

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
