Job Overview
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. Responsibilities
- Assess patients' needs and develop individualized care plans to meet their health goals.
- Coordinate with interdisciplinary teams to ensure comprehensive patient care.
- Manage the discharge planning process, ensuring patients receive appropriate follow-up care.
- Maintain accurate and up-to-date medical records for all patients.
- Provide education to patients and families regarding treatment plans and health management.
- Monitor patient progress and adjust care plans as necessary based on clinical outcomes.
- Advocate for patients’ needs within the healthcare system.
Skills
- Proficiency in managed care principles and practices.
- Strong understanding of medical terminology relevant to nursing and patient care.
