Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring the delivery of high-quality services, and facilitating communication between patients, families, and healthcare providers. You will primarily work with outpatient populations, focusing on various aspects of care including infectious disease management, geriatrics, and behavioral health.
Responsibilities
- Conduct comprehensive assessments of patients' medical histories and current health status.
- Monitor vital signs and document changes in medical records accurately.
- Develop and implement individualized care plans that address the specific needs of patients.
- Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
- Provide education and support to patients and their families regarding treatment options and health management.
- Collaborate with interdisciplinary teams to optimize patient outcomes and enhance service delivery.
- Advocate for patients' needs within the healthcare system while ensuring compliance with relevant regulations.
- Maintain up-to-date knowledge of best practices in case management and patient care.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in outpatient settings preferred; familiarity with hospital environments is a plus.
- Knowledge of infectious disease care protocols and geriatric patient considerations.
- Strong understanding of medical records management and documentation standards.
- Excellent communication skills with a focus on patient service and care coordination.
- Ability to work collaboratively within a multidisciplinary team while managing multiple priorities effectively.
- Previous experience in behavioral health is advantageous but not required.
