Nutritional consultant for the Shady Lane Nursing Home residents and staff as needed with regards to such issues including but not limited to:
Review of resident’s medical information in order to assess their nutritional needs and recommend a plan of care for them, initiate and implement/update care plans on all New/Re-Admissions by day five from admission date, Significant Changes, Quarterly and Annual Assessments.
Approval of Menu Monthly and make changes where needed to ensure nutritionally based meals according to resident needs, meet with Food Service Director weekly to go over menus.
Recommendations to the facility’s food service director and staff to provide nutritionally adequate and medically appropriate meals for residents.
Educate residents when needed regarding medically nutrition therapies that may enhance resident’s quality of life and care.
Educate staff quarterly on nutrition and nutritional intake of the long- term care and sub-acute rehab client. In-service calendar to be given to the DON at beginning of contract for the year of service.
Meet weekly with Food Service Director and Nurse Manager to go over concerns during normal business hours.
Submit consulting Dietician’s report to the following; Food Service Director, Director of Nursing, Nurse Manager, MDS Coordinator.
The ability to use Sigma Care System by recommending therapeutic diet orders and imputing documentation into the system for the residents of Shady Lane Home.
Survey requirements are to be on call to answer all questions and provide information as needed and to designate a day during the survey week for face to face with survey team prior to exit.
