Job Summary
We are seeking a compassionate and dedicated Medical Social Worker to join Southern Grace Hospice. The ideal candidate will play a vital role in providing support to patients and their families, helping them navigate the complexities of medical care and emotional challenges. This position requires a strong understanding of medical terminology, behavioral therapy, and the ability to work with diverse populations, including geriatrics and individuals with disabilities.
Responsibilities
- Conduct assessments to identify patients' social, emotional, and environmental needs.
- Develop and implement care plans in collaboration with healthcare professionals.
- Provide counseling services to patients and their families, focusing on coping strategies and emotional support.
- Assist patients in accessing community resources and services, including special education programs when necessary.
- Maintain accurate medical records and documentation of patient interactions.
- Facilitate communication between patients, families, and healthcare providers to ensure comprehensive care.
- Offer support in inpatient settings, addressing the unique challenges faced by hospitalized patients.
- Advocate for patients' rights and needs within the healthcare system.
Experience
- A degree in Social Work or a related field is required; licensure as a Medical Social Worker is preferred.
- Familiarity with medical terminology and experience working with medical records is essential.
- Previous experience in behavioral therapy, child family counseling, or geriatrics is highly desirable.
- Knowledge of special education resources and working with individuals with disabilities will be beneficial.
- Experience in inpatient care settings is an advantage.
- Proficiency in psychotherapy techniques is a plus.
