Job Title: Admissions/Discharge Coordinator
Department: Marketing
Supervisor: Administrator
SUMMARY: The primary purpose of the position is to consistently provide the highest quality of resident care available and establish a positive reputation in the community while delivering on the Eduro Healthcare Values of wellness, compassion, customer experience and company results.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Adherence to Policies
·Must show courtesy and respect to other employees, residents, family members, visitors, government agencies, the general public, etc.
·Abide by Eduro HealthCare’s drug and alcohol policy.
·Maintain good personal hygiene and dress and groom appropriately.
- Report on time as scheduled and follow all company policies and procedures.
- Attend staff meetings and in-service sessions.
- Become thoroughly familiar with emergency procedures.
- Must be able to perform duties in a timely fashion, and within the prescribed sequences and schedules.
- Must be able to perform assigned tasks with a minimum of supervision and develop awareness for and a willingness to perform other tasks, as needed, without supervision.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
Follow all established safety precautions when operating equipment, and report hazardous and defective equipment or conditions to your supervisor.
Must be cooperative with other departments and be courteous and respectful in dealing with them at all times.
Participate in and respond professionally to surveys (inspections) conducted by government agencies.
Supervisory Duties
Perform assigned administrative duties in accordance with established procedures and as directed by your supervisor, to assure that a successful, viable administrative program is maintained at all times.
Assume the authority, responsibility, and accountability necessary to carry out the duties of this department.
Admission Coordinator Duties
Each morning confirm any admissions or discharges from the previous day. Update the discharge list on the census.
Email the updated census out in the morning and upon any changes that may occur during the day.
Check voicemail regularly.
Attend management meeting daily.
Call on inquiries, pending admissions, etc. Pass admission inquiries on to DON for review as soon as they come in. Copy the face sheet and screened admit form and then pass it along to the business office for review of insurance coverage, payment status, etc.
Handle all inquiries professionally and according to established procedures, while providing appropriate information regarding the facility. Monitor and track all inquiries.
When final orders have been received, have DON review them and upon approval, set up the pick up time with the d/c planner at the hospital.
Call transportation to inform them of the pick up time. Notify admit nurses and hospital of admission.
In case of a large availability of beds, fax out our availability to hospital discharge planners
Sign in residents and families who are admitting. Make two copies of the documents, providing the original signed admit papers to the business office, and placing one of the copies in the chart and one copy with the medical records.
Conduct tours of the facility mentioning survey history, staffing ratios, therapy outcomes, etc.
Maintain a rapport with discharge planners to ease admission process.
Coordinate resident admissions, ensuring all requirements are met.
Discharge Coordinator Responsibilities
Research community support systems, social agencies, and family resources for adequate placement at time of resident discharge.
Develop and coordinate adequate discharge plan with patient, family/patient representative, interdisciplinary team, and community service entities.
Assist in obtaining and completing all necessary discharge paperwork and documentation

SUMMARY: The primary purpose of the position is to ensure the highest quality of resident care available, support staff and establish a positive reputation in the community while delivering on the Eduro HealthCare Values of wellness, compassion, customer experience and company results by providing rehabilitation services. The Social Services Assistant will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Service Department in accordance with current federal, state, and local standards, guidelines, and regulations, facility policies and procedures or as may be directed by the Administrator to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis. This position assumes the authority, responsibility, and accountability for carrying out duties in the Social Service Department.

Complete all social documentation in each resident medical record, including problems and goals. (Research problems and follow up on goals developed by the nurse case managers.)
Become familiar with the financial status of resident and family, and assist with S.S.I. application, Medicaid/Medicare applications and reviews, and other financial assistance, as needed.
Help with legal assistance, as needed
Coordinate Senior Companion Programs with Activity Director. Assist with other activities as needed or requested.
Maintain quality working relationships with all social service agencies in the area.
Research public support systems to benefit each resident.
Provide each resident with personal care items to assure their dignity and comfort.
Assist with phone calls, letter writing, shopping, etc.
Interview resident and family to obtain social history, and get their continued involvement in objectives and goals.
Assist with the social interaction of residents, staff, family members, etc.
Assist in community planning to improve services to the resident and family.
Work with Social Services Consultant and implement any recommended changes.
Maintain a reference library of written material, laws, etc. necessary for complying with current standards and regulations and that will provide assistance in maintaining a quality social service program.
Attend and participate in continuing education programs to stay professionally informed.
Ensure social service work areas are maintained in a clean and sanitary manner.
Order supplies and equipment for department as needed and use supplies in an efficient manner to avoid waste.
Provide consultation to medical staff, attending physician, and other resident care team members about resident or family needs.
Keep records and documentations up to date.
Patient Care
Be personally involved with all aspects of resident care to the extent necessary to assess its quality.
Respect all resident rights, including the confidentiality of resident care information.
Report immediately to the proper legal authorities if you have reason to believe a resident has been physically, emotionally, or sexually abused, or been a victim of theft of their personal property.
