Squirrel Hill Health Center, a Federally Qualified Health Center (FQHC), is seeking a dedicated and experienced Population Health Medical Assistant (PMA) to join our Clinical Operations Team. The PMA will play a crucial role in supporting patient care by scheduling appointments, relaying messages and lab results, participating in clinical huddles, and working on quality initiatives. This position requires collaboration with the entire SHHC team to ensure comprehensive and high-quality patient care. This position is full-time, Hybrid (office/home) after on site training.

Essential Duties:
- Appointments: Call patients to schedule follow-ups, immunizations, or in-house testing.
- Visit Pre-planning: Review patient charts, obtain discharge summaries, ensure completion of tests, and identify care gaps.
- Huddles: Print daily huddle, highlight key patient data, share pre-planning information, and request orders for immunizations or screenings.
- Quality Initiatives: Review care gaps, educate patients on preventive screenings, mail/fax necessary information, track outreach, assist with appointment scheduling, and upload reports to payor websites.
- Inbox Management: Manage Population Team inbox, review lab results with patients, direct clinical questions, address refill requests, and document patient interactions in the Electronic Health Record (EHR).
- Collaboration: Work with management, providers, RNs, and other SHHC teams to support population health quality initiatives and patient care plans.
- Excellent critical thinking and problem-solving skills
- Effective communication with diverse patient populations
- Strong interpersonal skills and courteous, respectful manner
- Efficient time management and organizational skills
- Ability to manage complex patients and situations
- Proficiency in EHR, Microsoft Word, Teams, Outlook, and Excel
- Minimum 3-5 years of experience as a Medical Assistant
- Certification as a Medical Assistant preferred
- Understanding of population health management goals
- Excellent organizational skills and attention to detail
- Knowledge of medical terminology, ICD-10, and CPT coding
- CPR certification
- Knowledge of a second language preferred
- Commitment to serving populations consistent with SHHC’s mission
- All CDC recommended vaccinations, including annual Covid and Flu, required
