SUMMARY: The Rural Aging Action Network (RAAN) Grant Service Coordinator will be instrumental in the success of the RAAN Grant, a 3-year initiative running from January 1, 2025, to December 31, 2027. The primary goal of this grant is to foster collaboration among service providers in Yellowstone County. The Service Coordinator will be responsible for overseeing project management, engaging with the community, and managing client relationships, both internally and externally. Additionally, they will occasionally provide respite care for caregivers, demonstrating a commitment to supporting the well-being of older adults and their families.
ESSENTIAL JOB FUNCTION: Project Management
- Engage in communication with program participants, RAAN staff, community partners, and other individuals as appropriate.
- Create or identify (in collaboration with the collaborative) an assessment tool to identify caregiver and client needs, addressing gaps in care, caregiving capacity and social determinants of health.
- Develop project delivery, tracking procedures, and follow-up for program staff.
- Maintain Client Relationship Management (CRM) database for volunteer/client management and project tracking/reporting.
- Disseminate project materials and information.
- Research and connect clients to resources to address identified needs throughout the project.
- Coordinate RAAN Grant meetings and action items.
- Ongoing check-ins with program staff.
- Other project-related duties as needed.
- Participate in GUIDE meetings/trainings.
ESSENTIAL JOB FUNCTION: Community Engagement
- Prioritize key volunteerism and senior network gatherings in service area and participate regularly with groups that share our mission.
- Be available to speak at community gatherings, civic organizations and churches to share the RAAN Grant opportunities for both potential volunteers, partner agencies and clients.
- Network with key referral sources for both clients and potential volunteers, including county and health plan case managers.
- Collaborate with Sales Representative and other RAAN service lines to identify and attend appropriate events that promote the grant activities.
ESSENTIAL JOB FUNCTION: Client Management
- Field phone calls from interested parties describing RAAN, finding most appropriate fit for client, referring to and working with partner sites as necessary.
- Take referral over phone and conduct in-person client assessments, working with case managers/referral sources and interested clients respectively.
- Maintain contact with clients, family of client, case managers, nurse care coordinators and partner sites, serving as liaison between various entities.
- Maintain client wait list and perform client and volunteer match meetings, including maintaining necessary wait lists and paperwork.
- Provide temporary relief to primary caregivers of older adults with disabilities or chronic illnesses. This involves assisting clients with daily living activities, providing companionship, and ensuring a safe and supportive environment.
- Maintain the confidentiality of all resident care information according to the St. John’s Policy on Confidentiality and Resident Rights.
REQUIREMENTS:
Minimum Requirements:
- Associate’s degree in healthcare, social services, or related field; or equivalent experience required.
- Must possess a current Montana State Nurses Aide Certification or have completed the SJU CNA Course and will obtain certification within 120 days of class completion.
- A valid driver’s license and a clean driving record are required.
Knowledge, skills, and abilities required:
- Ability to work with individuals and families from diverse backgrounds with care and compassion.
- Training in the area of computer skills, bookkeeping and accounts receivable, computer spreadsheets and data management, word processing, excellent communication and customer skills is required.
- Skills also include organizational skills, problem solving and the ability to work both independently and as a member of a team.
- Proven experience with detail-oriented tasks is required.
- Knowledge of resources available in program participant’s community preferred.
- Experience in program development or research is preferred.
- Experience managing team members is preferred.
- The position requires excellent communication, problem solving and interpersonal skills. The ability to interact effectively with persons from diverse cultures and backgrounds is highly important.
- The position requires broad knowledge of issues relating to aging, social services, and healthcare.
- The position requires excellent reasoning and analytical skills in order to understand and effectively interpret agency policies, procedures and regulations.
- The position requires creativity, flexibility, and adaptability to respond to work issues that can be ambiguous without an immediate clear-cut solution.
- The position may involve dealing with challenging behaviors and emotional situations.
PHYSICAL DEMANDS:
- Requires moderate physical work more than 70% of the time, including lifting up to 25 pounds regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and bending for prolonged periods of time.
- Communication requirements of the job would require the ability to hear and speak with others using the telephone, in person and in writing.
- The position requires the ability to move about, visit home and community living settings.
