St. Vincent's Hospital Westchester is seeking a Team Leader to join our expanding Care Coordination program. This program provides support and coordination services to adults in the community who struggle with medical and behavioral health conditions. Reporting to the Program Director, the Team Leader will supervise a new team of Community Health Workers who will assist clients with concrete needs, including access to transportation, housing support, and food and nutrition support.
Job responsibilities include:
- Provide supervision and support to a team of Community Health Workers.
- Ensure that program and regulatory standards are consistently met.
- Ensure that effective, high quality services are provided to assigned clients.
- Meet budget and productivity targets.
Qualified candidates will have excellent communication and organization skills.
Requirements: Master's degree in social work, psychology or a related field and 2-3 years' experience in a social service or health care setting, with at least one year in a supervisory role.
Pay rate: 72-78k
