Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position requires a strong understanding of social work principles, crisis management, and the ability to navigate various systems. The Case Manager will work collaboratively with clients to assess their needs, develop personalized care plans, and facilitate access to necessary resources.
Responsibilities
- Conduct thorough assessments of clients' needs and develop individualized service plans.
- Provide crisis intervention and support during critical situations, ensuring clients receive appropriate care.
- Collaborate with social services, and community organizations to coordinate services for clients.
- Monitor client progress and adjust care plans as necessary to achieve desired outcomes.
- Provide resources for behavioral health services, addiction counseling, and other relevant networks.
- Maintain accurate documentation of client interactions, progress notes, and service plans in compliance with agency policies.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
Requirements
- Training in Social Work or a related field with minimum 4-5 years experience.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
- Proficient in case management software .
- Ability to work independently as well as part of a multidisciplinary team.
- Strong organizational skills with attention to detail in documentation and reporting.
