Purpose of Classification:
The purpose of this classification is to deliver social services assistance, individual and group counseling to families, with a focus on non-emergency intervention. This includes assessments, intensive care management, counseling, and programming for clients who are self-directed in addressing problematic and/or unsafe behaviors, relationships, or living conditions.
Essential Functions:

Efficiently provide proactive case management services to clients by creating customized care plans and implementing evidence-based interventions. Emphasis is placed on prioritizing one-on-one and group time with clients due to limited access to high-quality counseling services for low-income clients in Cuyahoga County. The base productivity target is set at a minimum of 30% by the end of the fiscal year. (Productivity target is establish at least yearly)
Completes assessments; develops Individual Service Plans (ISP'S); acquires documentation; provides referral information, participates in performance planning and works toward professional and personal goals; maintains all service documentation in accordance with the policies and procedures of the agency.
Provides crisis intervention support for clients and maintains communications with staff, clients and referral sources; provides individual and group counseling; conducts agency business in an ethical manner, consistent with the National Association of Social Workers (NASW) Code of Ethics; engages and supports a diverse population of clients; maintains a minimum number of active cases.
Utilizes internal and external educational resources to provide lectures and workshops such as Facilitates Anger Management, Parenting, Domestic Violence, and Life Skills Workshops; Engages, educates and supports a diverse population of clients;
Engages community and businesses when appropriate to develop community resources to assist clients to become self-sufficient.
Foster a positive organizational culture that values diversity, equity, and inclusion, and promotes employee engagement and well-being.
Additional Functions:
Participates in the Case Manager's Continuous Quality Improvement (CQI) process, including team meetings and case load staffing; initiates regular home and field visits as part of an overall case management and outreach strategy.
Attends and participates in a variety of meetings, conferences, trainings, and workshops for professional development and to obtain best practices for improving knowledge, skills and abilities.
Actively develops and manages relationships at the grassroots level and ensures that Step Forward's brand and services are known in the community at governmental levels.
Actively cultivates strategic alliances with community organizations for profits and nonprofits —to build mutually beneficial partnerships.
Represent Step Forward at events, conferences, and other forums to promote its work and build relationships with key influencers.
Performs other duties as assigned.
Qualifications
Bachelor’s Degree in Social Work or a related field required; supplemented by one year of work experience in social work and knowledge in casework and advisory services; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job.
Must possess and maintain a Licensed Social Worker (LSW) designation or (LPN).

Salary : $39,842 - $49,803
