This position will typically be scheduled Monday through Friday
As a Care Navigator, you are a member of the dementia care interdisciplinary team. Where you are providing comprehensive care coordination services and advance care planning in order to maintain a long-term care partnership for patients living with dementia and their care partner in their homes. The Care Navigator will work collaboratively with the medical director providing oversight. Where you will provide comprehensive and integrated care with the goals of improving the quality of life for patients living with dementia and their care partner. You will help reduce caregiver burden, with the goal of supporting the patient and care partner, enabling the patient to remain in their home and community. The Care Navigator is trained in dementia assessment and care planning, providing a standardized set of services and extensive dementia and Alzheimer's education pertaining to the dementia and Alzheimer's disease process, for patients and their care partner(s).
Job Duties :
- Serves as a primary resource and contact to patients, who have dementia and Alzheimer's diagnosis, and their care partner(s) to assist in navigating the healthcare system.
- Prepares the patient and care partner(s) for the initial assessment evaluation process.
- Identifies medical and community resources available to the patient, assisting with referrals to other providers as indicated.
- Documents patient encounters and enters data into medical records to track progress and completion of domain activities.
- Develops a comprehensive person-centered care plan
- Act as the primary advocate contact for patients and their care partner
- Conducts in-home assessments, when appropriate, to better understand the patient and caregiver needs.
- Complete other duties as assigned.
