Key Responsibilities:
- Oversee and manage the facility’s MDS process ensuring timely and accurate resident assessments
- Collaborate with interdisciplinary teams to develop individualized care plans
- Analyze resident data for quality improvement and regulatory compliance
- Maintain accurate documentation and perform audits to support MDS accuracy
- Train and support staff on MDS procedures and best practices
- Serve as a resource for regulatory updates and ensure adherence to state and federal guidelines
Qualifications:
- Valid RN or LPN license
- Previous experience in MDS coordination or long-term care settings is preferred
- Strong analytical, organizational, and communication skills
- Detail-oriented with a commitment to quality resident care
