Synergy Health Partners is looking for a Physical Therapist to work in its Livonia, MI PT clinic. The Physical Therapist evaluates and plans treatment programs, administers and directs therapy for patients. Demonstrates knowledge of growth and development over the lifespan. Demonstrates the skills necessary for evaluating, interpreting age-specific needs and provides appropriate treatment. Acts under a physician’s prescription with regular consultations with the physician regarding treatment.
TYPICAL DUTIES AND RESPONSIBILITIES:
- Gathers data and performs patient evaluations including: objective data, assessment, goal setting and treatment planning.
- Provides prescribed and appropriate treatment, adhering to precautions, indications and contraindications.
- Maintains current documentation of all treatment interventions in the medical record as required by insurance and regulatory agencies. Actively manages and monitors the completion of any and all documentation required for patient care. Immediately follows-up with documentation requests from billing department or senior therapist. Responsible for timely completion and review of quarterly documentation peer reviews.
- Completes discharge planning based on assessment of patient status and post discharge needs.
- Effectively communicates with patients, families, physicians and colleagues with an emphasis on achievement of functional treatment goals and maintaining a positive work environment.
- Achieves and maintains targeted productivity as assigned to that cost center and area of service.
- Observes and respects patients’ rights to confidentiality, privacy and dignity.
- Evaluates and collaborates treatment plan with the physical therapist assistant in regards to establishing patient’s goal(s) via direct communications when changes/modifications are needed.
- Delegates tasks to physical therapy technicians and assumes responsibility and direct supervision for those tasks according to insurance and regulatory agencies.
- Maintains clinical competencies and professional growth through continuing education, mentoring and self-directed learning. Keep up-to-date with changes with licensure requirement and rules on supervision / rendering therapist. Understanding the difference between general and direct one-on-one supervision as it relates to different insurances.
- Actively participates in operational and quality initiatives and provides input and feedback to management.
- May supervise student clinical interns from affiliating professional programs and formally evaluate their performance.
- Participates in a collaborative identification and reporting of patient safety issues.
- Demonstrates clinical competencies, which are verified through observation, demonstration and/or testing that associates have the knowledge and skills necessary to provide care appropriate to the age of the patient.
MINIMUM QUALIFICATIONS
- Required Certifications, Registration or Licensure:
- License or eligibility for license in the State of Michigan.
- Minimum Knowledge and Education:
- Achieved entry-level degree from an accredited professional program.

Salary : $65,000 - $95,000
