Responsibilities
- Compounds and dispenses medication pursuant to a physician order.
- Monitors drug therapy for appropriateness of drug, dose, route, and duration of therapy.
- Screens for drug interactions, incompatibilities and contraindications.
- Recognizes potential adverse drug effects and intervenes as appropriate.
- Provides drug information to physicians, nurses, allied health professionals, and patients.
- Participates in interdisciplinary team conferences and other patient-care activities.
- Cares for all age group patients (birth and above).
Qualifications
Minimum Education
- B.S. or Pharm. D. degree from an accredited College of Pharmacy.
- Must provide evidence of meeting state mandated continuing education requirements upon request.
Minimum Work Experience
- Previous hospital pharmacy experience is highly desirable.
- Previous pharmacy computer experience desirable.
Required Licenses
- [Kentucky, United States] Pharmacists
- Licensed as a Registered Pharmacist by the Kentucky State Board of Pharmacy (or must be Kentucky licensed within 90 days of employment).
