Essential Duties for Personal Care Worker:
- Knowledgeable of the individualized Care Plan for clients, and provides support to clients according to their Care Plan
- Documents services provided in an accurate and timely manner in accordance with agency policy
- Report changes in the client’s condition or behavior to the Service Supervisor in a timely manner
- Notifies Service Supervisor when clarification or education of a task is required
- Fully understands all aspects of the client’s rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the Service Supervisor or management incidents or evidence of client abuse or violation of client’s rights
- Attends to the individual needs of clients, which may include assistance with grooming, bathing, oral hygiene, meal
preparations, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the client’s care requirements
- Assists in maintaining a safe, clean, and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry
- Performing other housekeeping tasks as indicated in the Care Plan
- Assists in grocery shopping as needed
- Assists and escorts to and from appointments / errands
- Assists and escorts to recreational events & outings. Participates in activities and functions as directed
- Takes client’s vital signs and recording input and output
- Communicates and interacts effectively and tactfully with clients, families, supervisor and other employees
- Work effectively as a team contributor on all assignments by providing specific information and observations of the
clients’ needs and preferences
- Submits timecards/ activity sheet in a timely manner (i.e. weekly)
- Implements appropriate safety and infection control practices
- Maintain client confidentiality at all times as required by HIPAA regulations
- Other duties as necessary or required
Job Types: Full-time, Contract
Pay: Up to $17.04 per hour
