Your work family includes doctors, nurses, (on call nurses, NP’s, triage nurses, admission nurses, LPN’s) social workers, CNA’s, chaplains, and volunteers, just to name a few. Qualifications:
- Licensed as a Social Worker through the Board of Georgia
- Possess effective communication skills (including strong verbal and written)
- Possesses a history of building and influencing patient and family relationships
- Holds a current drivers license, car insurance, CPR certification, and positive driving record.
- Hospital, home health, or hospice experience extremely preferred.
Responsibilities:
- Assess patient/family social and emotional factors to estimate their potential to cope with terminal
illness and death.
• Provide psychosocial counseling and support to the patient/family experiencing emotional, social,
and economic conflict.
• Participate in interdisciplinary team (IDT) meeting to establish, review, and revise the plan of care
for each patient, including exchange of information, review of problems, and sharing of
professional support.
• Maintain the dignity of the dying patient.
