Team Select is a leading provider of in-home healthcare services, dedicated to delivering compassionate and personalized care to our clients.
We are seeking a dedicated and compassionate Home Health Private Duty Nurse (RN) or (LPN) to join our team. The ideal candidate will have a strong clinical background and ***MUST HAVE GTUBE & OSTOMY CARE EXPERIENCE. MUST BE AVAILABLE FOR SHORT 6 HOUR SHIFTS***
Schedule:
5x6 hour shifts
8 AM - 2 PM
Monday through Friday
Key Responsibilities:
- Provide high-quality care.
- Adult Care Experience
- Monitor and assess patients' health conditions, including vital signs, and report any changes to the supervising physician.
- Administer medications, treatments, and other prescribed interventions.
- Ostomy Care
- Gtube
- Maintain accurate and timely documentation of patient care and progress.
- Collaborate with the healthcare team to develop and implement individualized care plans.
Qualifications:
- Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Arizona.
- Current CPR Certification
- Strong Clinical Skills
- Gtube and ostomy experience
- Reliable transportation and willingness to travel to patient home within the designated area.
Pay Rates:
LPN $28-$32 hourly
RN $35-$39 hourly
