ARP Case Manager Position
The Case Manager will provide comprehensive care coordination, advocacy, and support services for individuals experiencing substance use and mental health challenges. This role ensures clients access appropriate treatment, resources, and support systems to promote recovery and improve overall well-being.
Essential Job Functions:
· Work collaboratively with clients to assess their case management needs, goals, and barriers.
- Coordinate discharge planning and connect clients with services including therapy, medical care, housing, employment assistance, and peer support groups.
- Coordinate care among multidisciplinary teams, including therapists, psychiatrists, and community resources.
- Serve as a primary point of contact for clients, advocating for their needs within the facility and the broader community.
- Maintain accurate and timely case notes and other required documentation in compliance with HIPAA and facility policies.
- Prepare and submit regular progress reports to supervisors and external stakeholders as required.
- Educate clients and families about recovery strategies, relapse prevention, and mental health management.
- Participate in regular team meetings, case conferences, and training sessions.
- Collaborate with community partners and referral sources to enhance service delivery.
Qualifications
- Bachelor’s degree in Social Work or related field plus one year of experience in the field.
- SAC or SAC-IT, preferred
- Valid Driver's License
- Pass a criminal background check and be free of offenses prohibiting employment under HSF12 at the time of employment, and annually thereafter.
- Job Knowledge: Must have knowledge and understanding of human behavior, social work theories and techniques as well as community resources. Must be flexible and able to work with the uniqueness of each client. Must be sensitive to cultural differences within the agency and community.
- Fluent in Microsoft Office Applications (Word & Outlook)
