Job Title: Licensed Practical Nurse (LPN) / Charge Nurse
NOTE: THIS IS NOT A REMOTE POSITION
General Overview:
The Licensed Practical Nurse (LPN) works with individuals with developmental disabilities within the agency, under the supervision of the Agency RN.
Essential Responsibilities:
- Schedule staff using the Makeshift app, assigning LPNs to individuals.
- Maintain employee records in collaboration with the RN Supervisor.
- Ensure proper documentation for RN Supervisor review (QuickMAR/Therap).
- Direct nurses to observe and chart individuals' vital signs, dress wounds, prepare equipment, and assist with personal hygiene.
- Monitor individuals' reactions to medication or treatment and report back to the RN to request modifications to the treatment plan.
- Report to the RN.
- Perform specific nursing duties as assigned.
- Support individuals in planning nutritious meals and assist those on special diets.
- Cooperate with family members and other team members regarding reasonable requests related to individual needs.
- Train staff on the medical needs of individuals as required.
- Dispose of hazardous materials safely.
- Ensure medications are documented on the MARs as prescribed by the physician and conduct bi-weekly audits of the MAR.
- Review the ISP and attend ISP meetings for individuals receiving nursing services.
- Ensure the practice of the ‘Eight Rights of Medication’.
- Oversee the services provided by LPNs.
- Observe, document, report, and take action to ensure the quality of services for individuals.
- Follow-up to promote the quality of life for individuals served.
- Actively participate in quality improvement plans.
- Ensure medications are stored according to guidelines.
- Ensure doctor’s appointments are followed, and follow-ups are completed as needed.
- Confer with physicians, RN, PAs, and NPs to ensure individuals’ health status complies with their suggestions, recommendations, and referrals.
- Perform other direct nursing duties as assigned by an RN.
- Provide transportation, if applicable (must meet DBHDD transportation requirements).
- Obtain required training as mandated by DBHDD & HFR.
- Complete ongoing training (minimum of 16 hours annually).
Qualifications:
- Fluent in written and spoken English, with basic computer skills
- Licensed LPN in the state of Georgia
- BCLS certified
- NCIC/DBHDD clearance
- Capable of lifting 50 pounds without assistance
- Maintains a current driver’s license
- Must demonstrate a positive attitude, flexibility, and the ability to work with people from diverse backgrounds
- Competent in nursing areas including, but not limited to: skin & wound care, administering G-tube feeding, assisting with ADLs, completing nursing assessments, medication administration, and taking and documenting vital signs
