Description

Position Summary:

The role of Community Partners Supervisor is responsible for development and management of the Community Partners team in individualizing community-based activities for participants based on individuals’ beliefs, interests, preference, and support needs. The Community Partners Supervisor will maximize inclusion of the participants in the community by leveraging community networking, support resources and their natural support networks through assistive/adaptive technologies.  The Community Partners Supervisor will establish professional and collaborative relationships with both internal and external stakeholders to maintain consistent, timely, and open lines of communication.

Position Responsibilities:

In a manner consistent with supporting The Arc NCR Mission Statement the Community Partners Supervisor shall do the following:

  • Collaborate with agency leadership to provide coaching, performance improvement plans, and offer overall support and guidance to team members emphasizing equity, care, and compassion.
  • Participate in hiring and onboarding process including submission of Peer Training schedule to HR.
  • Ensures team members understand their role, expectations, policies and procedures while equipping them with the tools and techniques to optimize resources in order to provide quality supports and developing community resources.
  • Assign and manage all routes and daily activities, stepping in when necessary to ensure adequate supports are maintained.
  • Facilitate department functions to include monthly meetings and check-ins, status changes and transitions, trainings, and incident reports as necessary.
  • Engages in a minimum of 1 site visit per week.
  • Attends monthly leadership meetings.
  • Reviews GPS Alerts from Transportation and schedules team members for corrective action, when needed.
  • Maintains confidential and accurate records for team members (i.e., attendance, goals completion, performance evaluations, etc.) and completes required documents in a timely manner.
  • Reviews and approves weekly timesheets and mileage forms.
  • Attends individual Pre-PCP meeting as necessary, participates in PCP signing meetings, and ensure staff is trained on new PCPs within 20 days after the PCP meeting and submit signatures to Case Coordinator.
  • Undertake other duties as assigned, including special projects and cross-program development. 
  • Respond promptly to QE reviews within the given deadline.
  • Ensure that the supported person’s plan is conducted as outlined in the written plan, programs, schedules, and other documentation.
  • Review PMOF book to ensure availability of current PMOF and/or scripts.
  • Cultivate and maintain mutually beneficial relationships with community members and organizations to achieve service and participant goals.
  • Monitors crisis situations, collaborates with authorized staff and supported person to adjust supports and the environment as needed while adhering to policies and regulations.
  • Develops, maintains, and effectively uses information management systems to evaluate service and program outcomes and standards.
  • Communicate with the Assistant Director via text or phone call of any urgent care visits, hospitalizations, vehicle accidents, major staffing issues or concerns and police, fire or ambulance involvement or any calls to 911.
  • Review goal documentation to ensure support of billing.
  • Monitor the accuracy and timeliness of attendance tracking and recording.
  • Review monthly expenses and reconcile monthly petty cash.
  • Collaborate with the Finance Department to ensure compliance with agency policies and procedures and achievement of budgetary goals related to the financial responsibilities of the day program.
  • Plan and implement day-to-day operations, activity schedules, and short-term objectives with optimal resources utilization and adherence to budget.

Qualifications


Education, Training and Experience:

  • High School Diploma is required; Associate’s or Bachelor’s in Human Services is preferred.
  • DSP I or must be able to complete within the first 90 days.
  • Two (2) years of supervisory experience.
  • Three (3) years of experience in the human services field.
  • Fluent in Microsoft Office Suite (Word, Power Point, Excel, Outlook, etc.).

Physical Demands and Work Environment:

The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties associated with this position, an employee (using approved agency lifting techniques) will be called upon to support and lift an adult with developmental and other disabilities; transfer a person to and from wheelchairs; to and from beds; on and off toilet seats; in and out of showers; and, in and out of vans or other vehicles. An employee in this position must be able to physically intervene (when appropriate and in an approved manner) to protect a person from causing harm to themselves or others and to safely evacuate the community residence/van/facility during an emergency. These duties may entail occasionally exerting 50-70 pounds of force by pulling, pushing, or lifting objects or people.  The employee will collaborate with people who possess diverse disabilities and may exhibit a spectrum of behaviors, potentially including assertive and unpredictable behavior.

Employees will regularly be required to stand; walk; sit; use hands (or hands and arms) to carry, finger, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop; crouch and kneel. Employees will also be regularly required to hear (with or without hearing devices), see (with or without corrective lenses), and speak.

Additional Requirements:

  • Access to reliable transportation.
  • Active valid driver’s license, a clean driving record, and no more than two (2) points.
  • Maintain proof of current automobile insurance.
  • Maintenance of all required trainings under DDA guidelines and The Arc Northern Chesapeake Region policies and procedures.
  • Flexibility of scheduling to meet agency and program needs.
  • Ability to obtain DOT Medical Card (Non-CDL) within thirty days (30) of employment.

Hourly Position:

Min. Annual Salary: 53,267.45

Max. Annual Salary: 55,857.45
