Key Responsibilities
Duties
This is an hourly contract (1099) role requiring up to 10-14 hours per month with an anticipated census of 20 patients. Duties include:
Remote Responsibilities (70% of the time):
- Managing clinical needs via phone or remote communication.
- Collaborating with an interdisciplinary team of nurses, social workers, chaplains, and therapists.
- Interdisciplinary Group (IDG) Meetings: Participate bi-weekly for approximately two hours
- Location: 8582 Katy Freeway, Ste 225, Houston, Texas 77024-1830
- Face-to-Face Patient Visits: Conduct eligibility certifications in West Houston, with a concentration of patients in specific facilities, including homes.
- The covered area spans from Cypress in the north to Sugarland in the south, following the path of Highway 99.
- Approximately 75% of the patients are concentrated in a primary facility located in the Energy Corridor of west Houston.
- Building rapport with referral sources.
- Competitive pay of $200 per hour.

Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
- Active, unencumbered medical license to practice in Texas.
- Controlled substances registration with state and federal authorities.
- Board certification is valued but not required.
- Board Certification in Hospice and Palliative Care is particularly desirable.
- Hospice or palliative care experience preferred.
- Valid driver’s license and reliable transportation with required agency liability insurance.
