Essential Duties :
- Administer and document medications, treatments and therapies as ordered by the physician.
- Assess, document and follow-up all aspects of the individual’s health care Communicate clinical assessment information effectively to both professional and non-professional individuals, including family, advocates, staff and the individuals
- Accompany and / or transport individuals to medical appointments as needed
Evaluate self-medication potential and make recommendation to inter-disciplinary team
Creates a working climate that provides growth and job satisfaction of all personnel

Licenses / Education / Experience :
Licensed Practical Nurse or Registered Nursing License in the state of Pennsylvania
Valid Driver's License (issued from the state of residence) and driving approval from the Archdiocese of Philadelphia, CPR / First Aid Certification
Language Ability :
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; ability to write routine reports and correspondence; ability to read and write in English
Math Ability :
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to perform these operations using units of American money and weight measurement, volume, and distance.
Reasoning Ability :
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills :
Ability to use / learn various databases reporting system (Care Tracker), Microsoft Outlook, Word, Excel

Physical Demands :

Pay for LPNs start at $33 / hr. for experience. $1 shift differentials are offered to staff working evening, overnight, and weekend shifts.
