Job Title: Outreach Worker
Report to: Outreach Coordinator
FLSA Status: Non-exempt
Job Summary: This position is primarily responsible for providing street-based outreach services to the unsheltered community in the Greater Richmond Area. Outreach workers are responsible for collaborating with Daily Planet providers and staff to promote a team-based approach to patient care and work closely with Greater Richmond Continuum of Care (GRCOC) partners as a member of the GRCOC outreach team. The position requires significant time spent in the field engaging and assessing the needs of those experiencing homelessness and connecting patients to clinic services and other mainstream resources, while using a housing-first approach to assist clients in getting off the street into more stable housing.
Job Requirements: Bachelor’s degree in social services or related field. 1 to 3 years of general case management experience required. Staff should be knowledgeable about and demonstrate basic competence in the following areas: harm reduction, participant centered care, and trauma-informed care. Staff are expected to actively demonstrate a high level of respect, concern, and attention to client needs. Staff must be able to work in environments where participants are living: outdoor locations including those that are hidden, near bridges, or in wooded areas. Staff are expected to be able to have basic computer literacy for email communication, utilize HMIS database, and write communication as needed to assist clients. Ideal candidate will be able and willing to work flexible hours dependent on the needs of the community (including nights and weekends).
